Dr. Anil Bajnath is a Board-Certified Family Physician, author of “The Longevity Equation,” and Adjunct Faculty at George Washington University School of Medicine. He is the CEO and founder of the Institute for Human Optimization Precision Medicine Research Group. He offers personalized healthcare services, including nutrition, genomics, biomarker testing, and lifestyle monitoring. His expertise lies in understanding unresolved illness and formulating care plans, while his research focuses on pharmacogenomics, nutrigenomics, and the microbiome sciences.

Register and Complete Forms

    1. If you are a current patient, please log in using your credentials and skip to Scheduling below
  2. After registration login to the patient portal
  3. Choose the “Questionnaires” on the side menu.
  4. All patients MUST fill out forms in RED
 

Add a Valid Credit Card

  1. Login to your portal
  2. Go to Invoices and Payments
  3. Go to Add Credit Card
  4. Enter credit card information and hit save card

**A credit card MUST be on file in order to schedule and have your appointment confirmed. If you do not have a valid credit card entered, your appointment will not be confirmed.

Scheduling

  1. Once registration and forms are completed and a valid credit card is added, visit “My Appointments” to schedule a your desired appointment date and time (Please keep in mind these appointments are done by phone or video and NOT in person)
  2. Under “Select Provider” use the drop-down menu to select Dr. Anil Bajnath
  3. Choose the appointment type
    1. Initial Consultation (First Visit) 60 Min $600 – Patients must book first time visit with Dr. Anil as Initial Consultation
    2. Follow Up Phone Consultation 60 min $500
    3. Follow Up Phone Consultation 45 min (Lab Reviews) $375
    4. Follow Up Phone 30 Min $300
    5. Follow Up Telemedicine 60 Min $500

Dr. Anil's Scope:

Dr. Anil’s Longevity program is focused on prevention to optimize healthspan (how long you remain disease-free) and strategies to help reverse or mitigate common chronic conditions. Remember, Dr. Anil is an adjunct to your primary care and specialist team and not a replacement for them. With that being said, some conditions will require you to have a specialist to directly manage, such as, but not limited to, type one (1) diabetes, Grave’s disease, infertility (female or male), pregnancy, human immunodeficiency virus, any end-stage disease, active cancer, eating disorders or disordered eating, other psychiatric conditions beyond mild depression or mild anxiety, severe long-haul Covid, acute or chronic Lyme disease, moderate to severe mold toxicity, CIRS (chronic inflammatory response syndrome).

No FMLA, disability, handicap, sick notes, surgical or dental clearance, work or school physicals. Paperwork is the responsibility of your primary care physician. They love doing lots of paperwork. But on a serious note, all of these forms require a physical evaluation, and filling them out without a proper assessment would constitute fraud! Schedule an in-person appointment with your PCP or occupational health specialist to have these forms completed with them.

Dr. Anil is not the place for emergency care or urgent evaluations and does not perform medical evaluations over email or portal messaging.
